Property & Housing Partnerships
Property & Housing Partnerships.
We help create stable, well-managed homes for adults who need long-term specialist support — working alongside the people who own and manage the property. You keep the asset; we provide the regulated operation that makes it work.
For housing providers, landlords and long-term property partners across Bristol, South Gloucestershire and surrounding areas.
Why the right property matters
The right support needs the right home.
The home and the support are inseparable. A well-chosen, well-managed property reduces distress, supports independence and creates the stability that makes long-term care genuinely possible.
Too often, housing and care are arranged separately — and that is where specialist placements fail. The wrong property, in the wrong location. Adaptations that were never made. A placement put together reactively, under pressure. A landlord and a care provider who never really worked as one. The result is instability: voids, breakdowns, and people moved on again.
Care Horizons works the opposite way. The home and the support are planned together, governed together, and held together for the long term — bringing the asset and the regulated operator who runs it well into one arrangement. That is what keeps a placement stable, and what protects your asset.
Who we work with
We work with property owners and housing providers of all kinds
Whether you own one suitable property or a portfolio, the starting point is the same: a confidential conversation about what the building could become.
What we bring to a partnership
A regulated, reliable operator — not just a tenant
Long-term, stable occupancy
Supported-living placements are measured in years, not months. We are not a short-let or a transient tenant.
A CQC-regulated, professionally governed service
Rated Good, with ISO 9001 and ISO 45001 systems behind every placement.
Experienced operational leadership
Independently led since 2017, with a settled management team and Director-level oversight of every case.
Established referral pathways
Working relationships with commissioners and social workers who place people into well-run, well-located homes.
Ongoing quality assurance and oversight
Daily recording, audit and the governance a property owner wants standing behind their asset.
Transparent, structured joint working
Clear roles, clear responsibilities, clear agreements — with families, commissioners and clinicians, and with you.
What a good partnership can look like
When the home and the support are planned together
In some situations, the right property is what prevents a person being placed far from their family, community and the people who know them. We work with property owners, housing providers and landlords to create stable, long-term homes for adults whose needs cannot be met through standard housing routes. When the housing, the support and the oversight are aligned from the start, people are far more likely to stay settled, avoid repeated placement breakdowns, and build independence over time — and the property stays occupied, cared for and stable.
What becomes possible
When the right home and the right support come together
Fewer placement breakdowns
Stability from the start means far fewer of the failures that move people on again and again.
People closer to family and community
The right local home prevents someone being placed far from the people and places that matter to them.
Reduced crisis and escalation
A settled home and a consistent team head off the spirals that lead to crisis services and emergency moves.
Greater independence over time
With stability in place, people build skills, confidence and a life — at their own pace.
A stable, well-cared-for asset
Long-term occupancy by a regulated operator who maintains the home and stays for years, not months.
Less disruption for everyone
Families, commissioners and you all benefit when a placement simply holds.
Every partnership is supported by clear communication, written plans, operational oversight and ongoing review. We work with housing and property partners across Bristol, South Gloucestershire and surrounding areas — the homes behind our supported living, learning disability, behavioural distress and hospital discharge support. You can see how this holds in practice in our real stories.
How it works
A straightforward, low-risk way to begin
A confidential conversation
You tell us about the property, or the plan, the location and what you are looking for. We tell you honestly whether it fits the kind of homes we run.
We assess suitability together
Location, layout, adaptations, and whether it can support the people we would place there. Not every building is right, and we will say so.
A clear, written arrangement
If it fits, we agree roles and responsibilities in writing. Care Horizons operates the support; you retain the property.
For longer-term property partners
Joint venture opportunities.
Care Horizons holds what most property owners cannot easily build on their own: referrals, a proven care model, operational systems, governance, and a growing regional network. Property partners bring the capital and the asset.
For owners and investors who want to go beyond a single home, we are open to longer-term joint ventures on the same principle: you provide the property; we provide the regulated operation that makes it work. If that is the conversation you want to have, we would be glad to explore it confidentially.
From Vierka
A building is only ever half the answer.
Over the years I have seen situations where the right support existed, but the right home did not — and the opposite, suitable housing without the specialist support a person needs to remain there successfully. Neither works on its own.
The strongest outcomes happen when housing, support and long-term planning are considered together, not separately. That is the half most property owners can’t build alone, and it is the half we bring, and we stay.
